| USA $400.00 Canada & Mexico $470.00 All Others $570.00 |
I am enclosing the proper full payment, or an initial installment of at least one-third(1/3). It is understood that the balance due shall be paid in maximum of two annual installments, each due on the anniversary date of this application.
In the event of delinquency, it is understood that the By-Laws of the Society will apply and at the option of the Society, this application may be terminated. Payments made toward Life Membership are not refundable. |
